1. Proof of eligibility to work in UK - Original passport/visa if required & biometric card

2. Proof of National Insurance Number i.e., Payslip, HMRC letter etc.

3. Proof of address (2 proof of address not older than 3 months at your current address e.g., bank statement/ Utility Bills/ Council letter.) Not mobile or hospital letters.

4. Enhanced DBS to be completed. Please bring along your debit card for payment. (Reference of Update service if applicable.) Details of address for the past 5 years required for new application.

5. 2 x Reference details (2 professional i.e., previous employer details of name, telephone & email, college teacher, etc.) or details of a previous colleague in the case of character reference. Please note we do not accept families’ reference.

6. Proof of drivers’ lincense where applicable.

7. Previous up to date training certificates in Health Care but not a necessity as we provide training.
